Enrolment

We would love to welcome your family to our school.

To apply for enrolment, please complete the Application for Enrolment form and email it to cliftonpark.ps@education.wa.edu.au, together with the following documents:

• Application for Enrolment Form
• Birth certificate
• Immunisation statement (ACIR / MyGov)
• Proof of address (for example, a copy of a utilities bill or driver’s licence)
